Look for the line that starts with: (In your php.ini file, if you have access)
display_errors

Simply change it to display_errors = On

Otherwise check your error log for errors.

The only problem I see is the method for blog visitors to save checklists, so they can return and edit their checklist. It could either use WordPress's system (which I don't think very many people use for their visitors) or have it ask them to create a username/password that is saved with the record, or a combination of both.
